Course Objectives:

  • Identify at least three factors used in determining whether to provide telehealth treatment to a client temporarily out of state
  • List three pathways to practice authorization in a state outside of your home state
  • Use a decision tree to determine whether or not to continue care with an existing client who travels temporarily to another state

Course Description:

In a mobile society where outpatient mental health care is often delivered via telehealth, therapists and clients often want to continue meeting while traveling. Inconsistent state rules about telehealth can make it difficult to know when you’re able to maintain clinical contact, and what compliance steps may be needed.

In this interactive session, you’ll learn about temporary practice carve-outs, interstate compacts, crisis care, and the other factors that can impact whether it is appropriate to continue treatment when you or a client travel outside of your home state. You’ll come away with a step-by-step list of tasks for such situations.

  1. Introductions, audience poll and discussion (5 min)
  2. Key factors in determining whether to provide care (10 min)
    1. General telehealth appropriateness
    2. Location of the client
    3. Location of the therapist
    4. Payor reimbursement policies (e.g., client insurance coverage)
    5. Therapist insurance coverage
    6. Immediate safety
  3. Methods for practicing in multiple states (10 min)
    1. Licensure in multiple states
    2. Interstate compacts
    3. Temporary practice carve-outs
  4. When a client is planning to travel (15 min)
    1. Breakdown of appropriate steps
  5. Crisis assessment and care (10 min)
  6. Q&A (10 min)

Benjamin E. Caldwell, PsyD is a California Licensed Marriage and Family Therapist and Adjunct Faculty for California State University Northridge. He is the author of Basics of California Law for LMFTs, LPCCs, and LCSWs, and lead author of AAMFT’s Best Practices in the Online Practice of Couple and Family Therapy.
